Appointments

We see all patients on an appointment basis, and ask that you call in advance so that we may reserve time for you. The office number is 814.452.4214 or 1.800.531.5812. We try to see all patients on time, and request that you extend the same courtesy to us. If you cannot keep an appointment, call to notify us immediately. Please try to give us 24 hour notice so that this time may be given to another patient.

Please be prompt for your appointment.

You are asked to arrive 1/2 hour prior to your procedure so that the nurse can obtain a medical history and prepare you for the test or procedure.

What to Bring to Your Appointment

  • Your insurance card
  • Physician referral forms if required by insurance
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent x-rays or appropriate records you may have

On your initial visit please be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.

Patient Instructions

Please carefully follow the pre test or pre-x-ray directions that were given to you when your procedure was scheduled. If you have questions or concerns, call 814.452.4214 or 1.800.531.5812 and the nurse will be glad to assist.

Pre-test directions may suggest that someone accompany you when your procedure is done.

  1. If you are to have sedation, plan to spend 1 to 2 hours for recovery. You will need to be driven home when released.
  2. If it is necessary for you to remain in recovery, a family member may remain with you in the room.
  3. We will notify the person waiting for you if there is any delay. Most procedures take 20 minutes to 2 hours to perform.

Emergency Care

We recognize that emergency situations do arise and we will do everything in our power to respond to your problems as quickly as possible. If an emergency arises while the office is closed, call 814.452.4214 or 1.800.531.5812. The answering service will promptly forward your message to the doctor who is on call.

Insurance and Payment Information

Allied Urology Associates is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.

If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

Important Reminders!

  • If you have any questions or concerns following your appointment please do not hesitate to call 814.452.4214 or 1.800.531.5812.
  • If you are a new patient, you will be asked to fill out a patient information form, please arrive 15 minutes earlier than your appointment time.
  • Bring your insurance cards and referral forms from your primary care physician. You will be asked to sign an Allied Urology Associates office consent form when you arrive.
  • Contact your insurance company to confirm payment for the procedure you may have, and the coverage for facility fees.
  • Any procedure that we offer, and is not covered under your insurance policy or if you do not have medical insurance, our office policy states that you must pay in full for the procedure before it is done.
